Transitional Living Social Worker

The Children’s Home Society of WV is hiring a Social Worker or Case Manager in our Transitional Living Program at our Parkersburg location. The Transitional Living Program provides services to youth/young adults who are experiencing homelessness and assists them with securing and maintaining safe housing and making a successful transition to adulthood.

What you will be doing:

  • Screen referrals and assess the youth’s appropriateness for the program.
  • Provide and coordinate services to meet the needs of each youth.
  • Record accurate and complete documentation, including referrals, intake forms, assessment tools, planning documents, progress notes, incident reports, etc.
  • Assist youth in setting specific, attainable, and measurable goals relevant to their needs.
  • Provide, teach, and demonstrate basic living skills such as home management, consumerism, purchasing, budgeting, meal preparation, etc., to program youth.
  • Establish and maintain a positive relationship with youth through honest communication and active listening. Provide encouragement and support through positive role modeling.
  • Be consistently available for consultation and support.
  • Provide transportation, when needed.
  • Provide meaningful observations of program youth and make appropriate recommendations.
  • Participate in appropriate meetings to explore gaps in services for this population.
  • Obtain emergency food, clothing, and shelter for youth when needed.
  • Serve as a liaison, when requested, between the youth and professionals.
  • Advocate for the youths’ rights and engage the community in awareness of the needs of this population.
  • Provide aftercare services in accordance with program guidelines.
  • Ensure all activities and services are conducted in a manner that is sensitive to and shows respect for the culture and ethnicity of our clients.

What you will need:

  • Minimum of a Bachelor’s degree in Social Work or related Human Services field.
  • Social Work License or eligibility preferred.
  • Demonstrated ability to work with youth and families.
  • Valid driver’s license and automobile insurance coverage.
  • Criminal Background Check and Child Abuse Registry Check.
  • Flexibility to work irregular hours on occasion, including evenings and weekends.
  • Willingness to comply with professional standards as specified in the NASW Code of Ethics.
