The following benefits are for employees who are hired or moved
into a full-time or part-time position. Temporary workers
(hourly paid) do not qualify for the following benefits.
Health Insurance

Covered by Highmark Blue Cross Blue Shield. Part-time employees must work 75% of a full-time schedule (30 hours or more per week). Must enroll at time of hire - or within 30 days of hire - after 30 day qualifying period - coverage begins on first of month following 30 day qualifying period. Premiums split 75% (Agency) - 25% (employee). There are two plans offered: Both High Deductible plans with Health Reimbursement Accounts (HRA). Premiums are listed below:

Life
Insurance/Disability |
Covered by MetLife Insurance Company - Part-time employees must work 75% of a full-time schedule (30 hours or more per week). Must enroll at time of hire - or within 30 days of hire - after 30 day qualifying period - coverage begins on first of month following 30 day qualifying period. Life Insurance coverage is equal to one year’s annual salary. Disability coverage could be 60% of monthly salary up to retirement age. Complete details are available through the plan summary. Premiums entirely paid by Agency.

Extended
Sick Leave (ESL) |
All regular employees earn
up to .5 day per month if full work schedule is met. Part-time
positions are pro-rated based on percentage of schedule.
May only be accessed for serious illness of employee, and
after satisfying 5 qualifying days of absence must request
and submit physician's documentation in writing.

Paid
Time Off (PTO) |
All full-time and part-time employees
earn Paid Time Off for every full month worked. This
time is used for any days requested in place of vacation,
illness, or holidays.

of status:

Agency Pension Profit-Sharing Program |
After one year service with CHS of WV, and having worked at least 1,000 hours in that year, an employee qualifies for the profit-sharing plan. After verifying qualifications, employee receives enrollment package for entry on the first of the month of the following quarter (January 1, April 1, July 1, and October 1). A contribution from the Agency will be made on a monthly basis at a rate announced by the Agency. Current rate (10/1/01) is 6% of wage paid. Vests 20% per year - takes total of 6 years of participation for 100% vesting. Can withdraw after termination.

Tax-Sheltered
Annuity - Of CHS of WV - (TSA) |
All employees may begin contributing to the Tax-Sheltered Annuity program at the beginning of any quarter and may withhold between 2%-20% of gross income. Investments are directed by the employee and the employee can only close the account after termination. |
Voluntary
Benefits |
All employees may enroll upon hiring for the voluntary insurance benefits below:
Dental and Vision Insurance through Citizen’s Security Life Insurance Company. Premiums are paid solely by the employee and are deducted from semi-monthly paychecks one month in advance and remitted to Citizen’s by CHS. Rates are as follows:

Colonial Supplemental Insurance is offered to all employees and premiums are paid solely by the employee and deducted from semi-monthly paychecks at the end of the month for the previous month. Premiums are remitted to Colonial by CHS and vary by employee and type of coverage.
